Fill out your contact information & check out.  Within 24 hours of booking you will receive a confirmation text or email (from a real human being in our office) confirming your appointment date/time and address.  If you do not receive this text/email, we did not receive your scheduling request and you'll need to contact us to finish scheduling an appointment. Your card will not be processed until you've received this confirmation text/email.
